The forms have been used and tested by hundreds of doulas.
5 extra copies of Client Contact Sheet: makes
it easy to start over if the client has not hired you. Simply
insert new Client Contact Sheet and your file is ready to go.
LEFT SIDE OF FILE: The Business
Expense Card: Inside cover, lower left corner
is an expense card with lines so you can track expenses incurred
with the client.
Client Contact Sheet: All you will need for the
initial client interview. Some of the information can be taken over
the phone. The Client Contact Sheet is intake/history/meeting scheduling.
The "Directions" portion at the bottom of the form are for driving
directions to the client's house or meeting site.
Prenatal Interview Notes: Sheets for taking notes
and jotting down your thoughts/observations of the visit. It's a
good idea to note what you discussed, videos watched, techniques
practiced, etc
Agreement/Contract: 2 forms have been provided.
1 marked "Doula Copy" and 1 marked "Client Copy." The client copy
is on top so it can be easily removed once a deal for services has
been reached. Two payment arrangements have been provided for you
at the bottom of the form. The left 'agreement' if you want to be
paid up front for your services. The right 'agreement' if you only
require a deposit with the balance to be paid after the birth. Simply
cross out the arrangement you do not need and fill in the spaces
for the agreement you choose to use. If there is information in
the contract you do not agree to or services mentioned you don't
wish to provide, again, simply cross it out.
Receipt for Payment of Doula Services: Two copies
attached. The Client Copy is on the bottom for easy removal.
RIGHT SIDE OF FILE: Prenatal Visits/Birth
Prenatal Interview: Used after the client has
hired you and the business part has been taken care of. It’s
a good idea to familiarize yourself with the interview before using
it for the first time. The interview has been modified and is up
to date with the most relevant questions to help you best understand
your clients birth histories, wishes or concerns. If you don’t
feel comfortable asking any of the questions don’t ask them.
There is space at the bottom of the interview to add you own questions.
The Prenatal Interview is not intended to be the full extent of
your prenatal visits with the client, but rather a starting point.
A Guideline To Your Birthplan: Of course the client
does not need to use this as their actual birth plan, however it
can be an excellent starting point or forum for you to discuss plans
for the birth and it is obviously well designed to act as the birth
plan itself.
Labor Notes: When the Prenatal Interview and the
Birth plan have been completed it is a good idea to move the Labor
Notes sheets to the front of the right side for easy access during
the labor/birth. These are for charting and note taking during the
labor/birth, which I find necessary when creating the detail birth
log/story for the client.
